Ganglion cyst, also known as a hygroma, is a benign swelling filled with fluid. Externally, a ganglion appears as a smooth, subcutaneous lump that is semi-soft to the touch and not highly mobile. These cysts are actually filled inside with a gelatinous substance. This gelatinous material is synovial fluid, which normally surrounds joints and tendons to protect them during movement. It is believed that due to pressure on the joints during work, activity, or an awkward movement, synovial fluid can leak out and form a ganglion cyst. Ganglions typically appear near joints and tendons, most commonly on the wrist and finger joints, but they can also occur around the elbow, ankle, or knee. Their size can vary from that of a pea to that of a golf ball.
Ganglion cysts are generally harmless but can cause pain and aesthetic concerns. Sometimes they disappear on their own, but the exact timing or certainty of this cannot be predicted. Aspiration of the synovial fluid from the cyst can reduce its size, but there is always a possibility of recurrence. For this reason, surgical removal of the ganglion is a more lasting solution.

Preparation
Ganglion cyst surgery is performed under local anesthesia, meaning there is no special preparation required. The patient comes on the day of the procedure without the need for special diagnostic tests, except for examination and surgeon’s recommendation. Before the procedure, if deemed necessary by the doctor, basic laboratory tests and a general health assessment may be performed.
Operation time
After administration of local or regional block anesthesia, depending on the location of the cyst (back of the hand, palm, or finger area), the ganglion is accessed through a skin incision above the cyst. The cyst is carefully removed entirely, and then, in order to drastically reduce the chance of recurrence, the origin site is closed – the connection to the joint capsule or tendon sheath. The procedure lasts between 20 and 45 minutes.
Recovery
After the procedure, most patients are discharged the same day and can soon return to their usual daily activities, according to their comfort and doctor’s advice. The first dressing change usually takes place on the second or third day after surgery, while stitches are typically removed after about two weeks. Most patients experience mild recovery without the need for long work absences, except when the operated hand is dominant or frequently used.
Safety precautions
After surgery, it is recommended to avoid activities that strain the operated hand, such as carrying heavy loads, repetitive movements of the hand and fingers, or leaning on the operated hand. The wound should remain dry and clean until stitches are removed. Elastic bandages can be used as needed for extra protection and to reduce swelling. In case of pain, swelling, redness, or discharge from the wound, the patient should contact their doctor. Although the origin of the cyst is surgically closed to reduce recurrence risk, due to the nature of ganglions, recurrence is still possible. Therefore, follow-up care is an important part of postoperative management.
