Operation Description

Guyon's Canal Syndrome (Gion Syndrome) is primarily treated with rest, avoiding repetitive movements that exert pressure on the nerve, wrist immobilization, physical therapy, the use of anti-inflammatory drugs to reduce pain and inflammation, and corticosteroid injections. However, when these methods do not lead to improvement, surgery is the best solution.

The goal is to free the nerve from the cause exerting pressure on it. Regarding Gion Syndrome, the symptoms that commonly occur include:

  • Tingling in the fingers, both the ring and little fingers, which can be occasional or constantly present.
  • Weakness in the hand and wrist, making it difficult to grasp objects or perform precise motor tasks.
  • Joint pain in the hand and the entire arm is described as sharp or dull and may worsen with certain activities.
  • Loss of coordination and agility in the hand and fingers, making tasks like writing or typing more challenging.
  • Swelling of the hand joints accompanied by tingling or a burning sensation.
  • Loss of muscle mass in the hand and wrist, occuring in advanced cases of Guyon's Canal syndrome.

Candidates

  • Individuals diagnosed with ulnar nerve compression
  • Individuals experiencing symptoms of Guyon's Canal Syndrome
  • Individuals with a ganglion cyst or lipoma causing nerve damage
  • Individuals for whom other conservative methods of treating Guyon's Canal syndrome have not been effective

Preparation

Before the surgical procedure, the doctor will examine all symptoms, identify the cause of compression, and assess the extent of nerve damage. Afterward, surgical intervention is arranged, typically performed under local anesthesia. It is advisable to quit smoking a month before the operation, and two weeks before, discontinuing aspirin and herbal supplements that may affect blood coagulation, potentially complicating the operative and postoperative course.

Operation time

Local anesthesia is administered to the wrist area to block the affected nerve of the hand. An incision is made on the inner side of the wrist, at the base of the palm. Soft tissues are accessed, where structures causing pressure on the nerve are cut or removed. The incision is stitched, concluding the intervention. The duration of the operation is between 45 and 60 minutes.

Recovery

The patient goes home after the intervention. On the second day, they come for dressing changes, and stitches are removed on the 14th day of recovery. During recovery, the patient needs to rest and use the operated hand as little as possible. Full recovery takes several months, and physical therapy is also part of the treatment to restore wrist and hand functionality as quickly and effectively as possible.

Safety precautions

In the first days of recovery, swelling and bruising of the operated hand are possible but quickly subside. Additionally, complications related to Guyon's Canal syndrome surgery, although rare, may include incomplete nerve decompression (incomplete relief of pressure) and the recurrence of symptoms associated with this syndrome.

Following preoperative and postoperative doctor's advice can help prevent any complications and facilitate the proper recovery of the operated hand.
